The program is Elevra Optimal Health Patient Messages, operated by Elevra Optimal Health, LLC, a provider-led telehealth clinic serving Missouri residents. Messages are sent from (636) 698-0639, which is also the phone number for Elevra Optimal Health communications.
The program has two separate categories of message. You choose each category independently, and you may opt in to one without opting in to the other.

Marketing consent is separate from non-marketing consent. Declining marketing messages, or opting out of them later, does not affect your appointment, account, and care-coordination messages.
Text messaging is not a prescribing channel. Prescriptions are issued only after evaluation by a licensed provider, and only when clinically appropriate.
Text messaging is not for emergencies. If you have a medical emergency, call 911.
SMS consent is optional. Consent to receive text messages is not a condition of purchase and is not a condition of care. Each category of message has its own separate consent.
You can opt in in these ways:
Both consent boxes are optional, are unchecked by default, and are not required in order to submit the form. Accepting our Terms & Conditions is not consent to receive text messages, and submitting a form without checking a consent box does not enroll you in recurring text messages.
By opting in you confirm that you are 18 or older and that you are the subscriber or authorized user of the mobile number you provide.
If you initiate a text conversation with Elevra Optimal Health, LLC at (636) 698-0639, you consent to receive responses related to that conversation. Initiating a text conversation does not enroll you in recurring marketing or promotional messages. Marketing and promotional messages require a separate opt-in.
Message frequency varies.
Message and data rates may apply. Elevra does not charge for text messages. Any charges come from your mobile carrier under your own plan.
Consent may be withdrawn at any time. Reply STOP to any message to cancel. You will receive one confirmation message, and after that we will send no further texts.
Replying STOP stops every text message from that number, both marketing and non-marketing. If you would rather stop only the marketing messages and keep your appointment and account messages, tell us and we will update your preferences instead.

Opting out stops the text messages only. It does not change your care plan or your enrollment.
Carriers are not liable for delayed or undelivered messages. Delivery depends on your carrier, your device, and your coverage, so we cannot promise that every message will arrive or arrive on time. If you are expecting a message and have not received it, call the clinic.
Mobile phone numbers and SMS opt-in consent will not be shared with or sold to third parties or affiliates for marketing or promotional purposes.
We share your mobile number only with the service providers that deliver our messages. Those service providers may process the information solely as necessary to perform services on Elevra's behalf, and not for their own purposes. Our Privacy Policy explains what we collect and how we use it.
Text messaging is not a secure channel. Please keep texts to scheduling and general questions, and do not send detailed medical information by text.

These terms cover text messages only. Consent to receive text messages is not consent to receive automated, prerecorded, or artificial-voice telephone calls, and we do not treat it as such. If we later use automated or artificial-voice calls, we will ask for that consent separately.
